Your Personal Wellness Self-Assessment
Explore the nine dimensions of wellness, recognize your strengths, and identify areas you may want to improve.
A Starting Point for Your Wellness Journey
Wellness involves more than physical health. Our emotions, relationships, finances, surroundings, nutrition, purpose, and everyday activities can all influence our quality of life.
This assessment offers a simple way to reflect on how satisfied you feel with different areas of your life.
There are 27 statements, with three questions for each of the nine wellness dimensions. Most people can complete the assessment in about 5–10 minutes.
Your answers are processed entirely in your browser. This page does not transmit your responses to a server or automatically store them.
You may print or copy your results if you choose. If you close or refresh the page, your answers will not be preserved.
How to Answer
For each statement, select the number that best reflects your experience during the past month.
| Rating | Meaning |
|---|---|
| 1 | Strongly disagree |
| 2 | Disagree |
| 3 | Neither agree nor disagree |
| 4 | Agree |
| 5 | Strongly agree |
There are no right or wrong answers. Answer according to your own circumstances, not what you think someone else expects.
This is an educational reflection tool, not a clinical assessment or validated measure of physical or mental health. Ratings reflect your own perceptions and do not diagnose medical conditions.
Your Wellness Assessment Results
These results summarize your personal ratings across the nine dimensions. They are intended to encourage reflection, not to judge your health or predict outcomes.
All nine dimensions are weighted equally. Each dimension is the average of its three statements. The overall rating is the average of all 27 responses. These cutoffs are descriptive only, not validated clinical thresholds.
Your Nine-Dimension Wellness Profile
Longer bars represent higher self-reported satisfaction or agreement with the statements in that dimension.
Scores range from 1 to 5. A lower rating is not a diagnosis, and a higher rating does not guarantee good health.
Areas You May Want to Explore
The following suggestions are based on your lowest-rated dimensions. They are possibilities, not requirements. Choose what matters most to you.
Recognizing Your Strengths
Wellness planning should include what is already working well, not just areas of concern.
Your Next Step
Consider choosing one dimension and one realistic action to begin. You do not need to change everything at once.
Your assessment results can help guide the priorities you choose in your personal wellness plan.
The wellness plan page opens separately. Use the Copy Results button to take your scores and suggested priorities with you. No responses are automatically transferred.
